You must have a diagnosis of diabetes mellitus and at least one qualifying foot condition, such as peripheral neuropathy with evidence of callus formation or other qualifying foot problems.
You must be under a comprehensive diabetes care plan. Therapeutic shoes or inserts must be prescribed and certified as medically necessary by a qualified provider. The shoes and inserts must be obtained from a certified supplier.
The physician managing your diabetes must certify that therapeutic shoes are medically necessary and that you are being treated under a comprehensive care plan.
